Passport:

Original Passport with the validity of minimum six months from the date of return and minimum one blank page for visa stamping.*Attach all old passports (if any).*

Israel Visa Application form:

• One visa application form to be duly filled and signed by the applicant.
• An online application form should also be filled through the following link https://www.israelvisa.in/visalive/online/onlinevisaform.aspx . Photograph and ID proof (Adhaar Card, PAN card, Driving license or others) should be uploaded while filling the form. Post filling the online form, a printout of the same should to be taken and has to be attached with manual form (duly filled and signed by the applicant) and shall be submitted at the Israel visa application centre along with the other required documents.

Following forms also need to be filled, signed and submitted along with the application:

• Checklist

Photo Specification:

Two recent passport size photographs with matt or semi matt finish, 70% face coverage, white background and without border (Size: 55 mm X 55 mm).

Please note: Photograph should not be more than 6 months old, scanned/stapled and should not be used in any of the previous visas.

Covering Letter:

Covering-Letter from applicant mentioning his name, designation, passport number, purpose & duration of visit and travel itinerary in brief. The letter should be duly signed by the applicant & addressed to – The Visa Officer,Embassy of Israel, New Delhi. Please Note : Covering-Letter on Letterhead if applicant is Self Employed or on plain paper if Employed.

Financials:

• Original Personal bank statement for last 3 months with Bank stamp & seal and applicant's name must be mentioned on the statement. Please Note: Internet bank statement will not be accepted.
• Salary slips for last 3 months.

Proof of Occupation:

Original of Invitation from the Ukrainian citizen issued by the local foreigners registration authorities or ORIGINAL of the INVITATION , issued by one of the medical institutions of Ukraine if the person goes for treatment to Ukraine

Employed

• No Objection Certificate from employer mentioning the travel dates.

Student

• Leave Sanction Letter/ NOC from school mentioning the travel dates along with school ID card copy.

Proof of Accommodation:

Hotel booking along with Day-to-Day itinerary from the tour operator in Israel.

Certificates:

If applicant is traveling along with the family, a copy of marriage certificate & minor's birth certificate will have to be submitted along with the application. Please Note: If spouse name is not endorsed in the passport then an original marriage certificate duly appostilled from the Ministry of External Affairs, Patiala House is required.

Medical:

Original Medical Insurance for the entire period of stay amounting US$ 40,000 or equivalent. Please Note: The policy has to clearly specify the period of validity.

Authorization Letter:

Applicant will have to submit an authorization-letter in the name of Mr. Ravi Swamy, Mr. Nitin, Mr. Ramakant and Mr. Deepak Kumar to submit and collect the passport.

Note:

• In case applicant is visiting to any relative or friend, an invitation Letter from invitee in Hebrew or English language stating the applicant's name, passport number, purpose & duration of stay and accommodation details along with sponsorship details is required. The letter should be duly signed by the invitee mentioning his name & contact details and invitee's passport and visa copy should be attached.
• Applicant has to submit copies of his/her Aadhaar Card & Voter Id.
• All documents submitted should be on A4 size paper.
  Please Note: If the original document is on a different sized paper then the photocopy should be on A4 sheet.
• One Photocopy set of all documents mentioned above in the requirements have to be submitted along with application (apart from Passport).
• Embassy may ask for any additional documents or can call for the interview.
